Comparative analysis of publicly reported health care data to patient safety culture in top performing hospitals

Abstract:

Since the release of the two Institute of Medicine (IOM) reports, “To err is human, building a safer health system” (Kohn, Corrigan & Donaldson, 1999) and “Crossing the quality chasm: A new health system for the 21st century” (Kenney, 2008), increased emphasis on patient safety in healthcare facilities has been the focus for the industry. In April 2005, the Centers for Medicare and Medicaid Services (CMS) began publishing information regarding hospital performance and ranking based on measures regarding infections, mortality and readmission rates on a website called Hospital Compare. Since 2005, additional organizations providing publicly reported data have been formed. Two goals have been identified for measuring and reporting hospital performance: 1) provide consumers with information they need to choose a hospital where they will receive high quality care and 2) spur improvement in quality over time (Werner & Bradlow, 2010). However, since this performance rating is based solely on the hospital's coded information and claims data, the information may not accurately reflect the true performance of the hospital, particularly the hospital's patient safety culture. This study provides a comparative analysis of the publicly reported healthcare data to patient safety culture in top performing United States hospitals. This research used HealthGrades 2010 report that named 50 top performing hospitals. Using a mixed methods approach, I was able to select five top performing hospitals from this sample and conduct on-site evaluations in order to determine if there was a correlation to the publicly reported data and the hospital's patient safety culture.
