Clinical insights in glaucoma from the Los Angeles Latino Eye Study

Abstract:

Purpose. To compare intraocular pressure (IOP) by Goldmann applanation tonometry (GAT), dynamic contour tonometry (DCT) and examine effects of central corneal thickness (CCT), corneal curvature and IOP.

Methods. This cross-sectional study of 2157 participants in the Los Angeles Latino Eye Study compared GAT to DCT with respect to CCT, corneal curvature, and IOP.

Results. Mean GAT IOP was lower (14.4±3.2 mmHg) than DCT (16.0±3.6) (p<0.0001). Both increased with CCT, an effect that was greater with GAT (p<0.0001) than DCT (p=0.0012). Adjusting for CCT, corneal curvature affected DCT IOP (p=0.02) but not GAT (p=0.3), and the greatest difference between GAT and DCT was at low IOP levels (3.55±3.1) (p<0.0001).

Conclusions. IOP by GAT was lower than DCT, the difference greater with thinner CCT. DCT was less affected by variations in CCT. Corneal curvature affected DCT but not GAT. GAT underestimated IOP at higher levels and overestimated at lower levels compared to DCT.
